This is a 1999-P (Philadelphia) Jefferson Nickel Broadstruck Mint Error NGC MS67 FS (Mint State 67 Full Steps). The broad strike makes the coin look like the outer areas of the coin are pushed outward and stretched. The coin does have some yellowish tone on the obverse and reverse with the reverse also having like a cloudy look in certain areas. The plastic holder itself has some marks due to shelf life (it's an older NGC slab), but they are really only noticeable when it reflects in the light.
